Male Infertility

Male infertility is means a man that lowers the chances of his female partner getting pregnant. These main types are:

Oligospermia – Low Sperm Count

Azoospermia – Nil Sperm Count

Asthenospermia Low Sperm Motility

Immunonologic Infertility–  A man’s body makes antibodies due to injury, surgery or infection that attack his own sperm

Retrograde Ejaculation– Semen goes backwards in the body that means they go into bladder instead of out the penis

Hormones – Hormones made by the pituitary gland tell the testicles to make sperm. Very low hormone levels cause poor sperm growth

Chromosomes – Sperm carry half of the DNA to the egg. Changes in the number and structure of chromosomes can affect fertility. For example, the male Y chromosome may be missing parts.

Medication – Certain medications can change sperm production, function and delivery. These medications are most often given to treat health problems like:

Each of these conditions requires detailed evaluation and proper treatment. Male Infertility if properly treated can get fantastic results.

Micro-TESE

Microsurgical testicular sperm extraction (microTESE) is a surgical procedure used to retrieve sperm from inside a man’s testes (organs that produce sperm)

It is used for men with non-obstructive azoospermia, which is when a man cannot produce enough sperm to have a detectable amount in his semen, a common cause of male infertility.

  • Patients with azoospermia
  • Patients with severe above male factor infertility
  • Patients with obstruction of the vas deferens of surgical cause (vasectomy)
  • Patients with absence of the vas deferens of genetic cause (mutations of the cystic fibrosis gene)
  • Patients with obstruction of the vas deferens of infectious cause
  • If a man has an adequate level of testosterone in his blood after a hormone test and has other tests that indicate that his testicles are not producing normal amounts of sperm
  • Whether a man remains azoospermia even though he has received treatment and his testosterone levels have been normal for at least four months

Benefits of Micro TESE:
Why Micro TESE is better than a biopsy?

  • Micro-TESE is usually reserved for non-obstructive azoopermia, for those who have already had a fine needle aspiration or biopsy without recovering sperm. However, there is scientific evidence to show that for men with non-obstructive azoospermia, fine needle aspiration or biopsy before micro TESE may be not recommended.
  • The success rate of Micro testicular sperm extraction (Micro-TESE) is quite favorable for men. In fact, doctors can find sperm about 60 % of the time during micro TESE procedures.
  • Many studies have also shown that Micro- TESE procedures yields the highest rate of sperm retrieval and causes the least amount of damage to the testicles.
  • ICSI with Micro testicular sperm extraction (Micro-TESE) allows to solve many cases of absence of sperm in the ejaculate and severe male factor infertility, helping many couples in fulfilling their parenthood dreams.

How is Micro-TESE Performed?

Micro TESE procedure is performed in the following cases-

  • Sperm and testosterone (male hormone) are produced in the 2 testicles of the scrotum.
  • Sperm leave the testicles through a rolled tube called the epididymis. There they remain until they are ready to be used.
  • Each epididymis is attached to the prostate by a tube called the vas deferens. This duct runs from the scrotum to the groin, then the pelvis and behind the bladder.
  • There, each vas deferens joins the seminal vesicle and forms the ejaculatory duct.
  • For ejaculation, sperm pass through the ejaculatory ducts to mix with fluid from the seminal vesicles, prostate, and other glands to form semen.
  • Semen travels through the urethra and exits through the end of the penis

Micro-TESE procedure is performed when the goal is pregnancy but is not possible without help. It is for men who have little or no sperm in semen, or men who are not able to ejaculate.

In these cases, sperm can be collected from other parts of the reproductive tract. To obtain better pregnancy rates, Micro TESE process is used with in IVF and intracytoplasmic sperm injection (ICSI).
